Residential Treatment - ASAM Levels 3.5 & 3.1
What It Is
Our residential treatment program provides 24-hour, structured care for adults struggling with substance use disorder. Clients live on-site at our Prestonsburg facility and participate in a full daily schedule of therapeutic, educational, and community activities -- all within a safe, supportive, faith-centered environment.
Our residential program operates at two levels of care:
ASAM Level 3.5 — Clinically Managed High-Intensity Residential Services: Our primary level of care, designed for individuals who require the highest level of structure and support to begin their recovery.
ASAM Level 3.1 — Clinically Managed Low-Intensity Residential Services: Available for individuals who are progressing in their recovery and are ready for a less intensive residential setting, while still benefiting from the stability and community of on-site living. At this level, clients begin to re-engage with the outside world in structured, supervised ways — including attending 12-step meetings, religious services, and approved community activities. This gradual reintegration builds independence and real-world accountability while the client remains anchored in the safety and support of residential care.
Both levels are housed within the same facility, allowing clients to step down from 3.5 to 3.1 as they progress — without disrupting the relationships, routines, and community they have built during treatment.
Who It's For
Residential treatment is appropriate for adults who require 24-hour support and supervision, who have not been successful in lower levels of care, or who are entering treatment for the first time and need a strong, structured foundation for recovery. Admission to 3.5 or 3.1 is determined through a comprehensive clinical assessment at intake.

Dual Diagnosis Treatment
What It Is
Many individuals who struggle with addiction are also living with mental health conditions such as depression, anxiety, PTSD, or bipolar disorder. When these conditions go unaddressed, they can drive substance use and undermine recovery. Our dual diagnosis program provides integrated treatment that addresses both conditions at the same time — because treating one without the other rarely leads to lasting results.
Who It's For
Dual diagnosis treatment is for individuals who are dealing with both a substance use disorder and an underlying mental health condition. This includes people who have been previously diagnosed with a mental health disorder, as well as those who have never been formally evaluated but are experiencing symptoms that may be contributing to their substance use.

Individual & Group Therapy
What It Is
Therapy is at the heart of everything we do at Fresh Start. Every client participates in both individual and group therapy throughout their time in our program — two distinct but complementary approaches that together create the foundation for lasting recovery.
Individual Therapy
One-on-one sessions with a licensed clinician give each client the space to explore the root causes of their addiction, process trauma and loss, identify patterns of thinking and behavior that have contributed to their substance use, and build the coping skills and self-awareness that recovery requires. All individual therapy at Fresh Start is delivered through evidence-based approaches including Cognitive Behavioral Therapy (CBT) and Motivational Interviewing (MI).
Group Therapy
Group therapy is one of the most powerful tools in addiction recovery. Our daily group sessions provide peer support, shared accountability, and the opportunity to learn from others who are walking the same path. Facilitated by licensed clinicians, our groups cover a range of topics including relapse prevention, emotional regulation, healthy relationships, grief and loss, and faith and spirituality.
Who It's For
All clients enrolled in our residential program participate in both individual and group therapy as part of their individualized treatment plan. Frequency and focus are tailored to each person's specific needs and progress.

Family Support Services
What It Is
When someone struggles with addiction, the people who love them struggle too. Spouses, parents, children, and close friends often carry their own pain, confusion, and exhaustion — and that weight deserves to be addressed. Our family support services are designed to bring families into the healing process, rebuild trust and communication, and equip loved ones with the tools they need to be a healthy, sustainable part of their family member's recovery.
Who It's For
Family support services are available to the spouses, parents, children, and close loved ones of clients enrolled in our program. We welcome family members at all stages — whether they are just learning about addiction, have been living with it for years, or are somewhere in between.

Outpatient Services

Our outpatient services are currently in development and will be available soon. We will offer two levels of outpatient care: Intensive Outpatient Program (IOP) — ASAM Level 2.1: Designed for individuals who have completed residential treatment or who do not require 24-hour care but benefit from a structured, clinically intensive schedule. Clients attend treatment sessions multiple days per week while living at home or in a supportive living environment. Outpatient Services — ASAM Level 1.0: A flexible, lower-intensity outpatient program designed to support clients in the later transitional phases of recovery — including those in sober living arrangements — as well as members of the broader community who are seeking ongoing support. This level of care is open to both Fresh Start program graduates and the public. Contact us to be added to our notification list or to discuss whether outpatient services may be the right fit.

Recovery does not end when treatment does — and neither does our commitment to the people we serve. Our Aftercare & Alumni Support program is currently in development and will provide graduates of our residential program with ongoing peer community, relapse prevention resources, check-ins, and recovery coaching to help them sustain and build on the progress they worked so hard to achieve.
